Part Time Remote Nursing Jobs in Maryland: This is a remote function. CircleLink Health seeks enthusiastic, tech-savvy registered nurses to serve patients enrolled in Medicare’s Chronic Care Management Program while working remotely. In this part-time (20–25 hours per week) position, an RN Care Coach will be assigned a group of patients to monitor and contact each month. During these monthly calls, you will educate, coordinate care, close preventive care gaps, and advise on self-management strategies to keep patients out of the hospital.

Key Responsibilities:

  • Utilize our specialized care management software to make monthly phone calls to Medicare patients with two or more chronic conditions (diabetes, CHF, chronic pain, COPD, etc.).
  • Establish and maintain rapport with patients to assist them in achieving enhanced health through SMART goals and self-management education.
  • Implement and enhance the Plan of Care by updating the patient’s medications, upcoming appointments, biometrics, symptoms, and interventions.
  • Connect the patient as required with community resources, such as transportation, personal care needs, prescription/DME assistance, and social services.
  • Perform transitional care management activities on high-risk patients discharged from the hospital and the emergency room in order to prevent avoidable readmissions.
  • Close care gaps by promoting and assisting preventive care measures, such as annual checkups, vaccinations, cancer screenings, follow-up/specialist consultations, etc.

Scheduling and Other Requirements

  • RN requires a RELIABLE internet-connected computer
  • Minimum availability of 20 to 25 hours per week is required.
  • Using our software, you will commit to your own agenda.
  • Work must be completed on WEEKDAYS from 9 a.m. to 6 p.m.
  • This is a 1099, non-expiring contract position.
  • Care instructors are responsible for their own insurance and taxes.

Compensation:

Monthly compensation is equal to $15.00 per initial clinical encounter per patient. A clinical encounter occurs when two conditions are met: the patient has a successful clinical call, and their chart timer contains at least 20 minutes.

  • Example: If in one hour you called and spoke with two patients and spent 30 minutes with each, your hourly rate would be $30.00 ($15.00 per point reached multiplied by two).

In addition, coaches will receive $6.00 per patient call if the patient does not answer and $4.00 per patient call if the patient withdraws from the program upon responding.

In one hour, if you made one 20-minute successful call, attempted to reach two patients who did not answer, and then made another 20-minute successful call, your total hourly rate would be $42. See details below:$30 for two effective 20-minute visits, and

  • $12.00 for two unanswered messages to two patients is equivalent to $42.00 per hour.

Pay Timing:

Monthly through direct deposit, forty days after the last day of service. This is due to the time it takes Medicare to process reimbursements, but your monthly payment is guaranteed following the end of the month.
